For those who: have noticed — in their own treatment, in the treatment of someone they love, in clinical work — that much of contemporary care is structured around silencing symptoms rather than hearing them; who want the substantive procedure for hearing what the symptom is saying before deciding what the treatment should address.
Not for those who: are in acute clinical crisis. The substantive hearing work is not the right unit in acute crisis; the right unit is acute clinical engagement. Return to the brochure when the acute situation has stabilised.
What this is — the clinical reality
This is the second brochure of Bundle 11. The foundation brochure named the central conflation in disorder discourse — substrate-disorder and intelligent-response called by the one name «disorder». This brochure addresses the central operating distinction that runs through both kinds: the distinction between treating the symptom as information about what is wrong and treating the symptom as noise to be silenced. The distinction operates whether the underlying problem is substrate-disorder (the symptom signals the substrate that needs intervention) or intelligent-response (the symptom signals the situation that needs to change). The clinical work in both cases starts with hearing what the symptom is saying.
The brochure is for the reader who has noticed — in their own treatment, in the treatment of someone they love, in clinical work — that much of contemporary care is structured around silencing symptoms rather than hearing them. The headache that gets a pill rather than a question. The anxiety that gets an SSRI rather than an inventory of what changed. The insomnia that gets a sleep aid rather than an examination of the evening configuration. The pattern is not always wrong; sometimes silencing the symptom is the right intervention, especially in acute clinical situations. But the silencing-without-hearing has become the default, and the cost the default produces is what the brochure addresses.
A note on substrate before we go further. The book substrate for this brochure is extremely strong. «Psychosomatics Without Esotericism» contains the dedicated Epilogue «Illness as Signal, Not Sentence» that names the brochure's title-concept directly; the «Ten Signals of the Body» chapter that operationalises the hearing work; the sentry-focus chapter that explains the mechanism the not-hearing produces; the weakest-link principle that grounds the personalised attention; the treating-effect-not-cause passage that names the cost of silencing without hearing. «Above the Clouds There Is Always Sun» supplies the body-dissociation passage that names the extreme of not-hearing (cross-cited from 11.1). «Three Pencils. Extension» supplies Chapter 1 «A Thermometer for Thinking» — the parallel diagnostic-instrument framing applied here to symptoms (cross-cited from 10.3). The substrate is the strongest of Bundle 11 so far; the brochure draws all three forward.
