Health Anxiety

Vitamodo School · Bundle 8: Anxiety & Rumination · Brochure 6 of 10 · Version 1.0

Andris Saulitis, MD

For those who: have been monitoring the body — feeling for lumps, checking pulse, watching skin, reading medical articles to find their symptoms named — and have noticed that each medical reassurance produces brief relief before the next symptom or article restarts the pattern.

Not for those who: are confronting an actual medical investigation in progress. If you have new neurological symptoms, chest pain, or another concrete clinical sign that warrants medical assessment, get the assessment first; the brochure addresses the cognitive pattern that persists after the substrate has been examined and ruled out.

What this is — the clinical reality

This is the sixth brochure of Bundle 8. The first five addressed anxiety as a state, fear vs worry, the three-pencils method on anxious worry, rumination, and catastrophizing as a general structure. This brochure addresses health anxiety — the specific clinical syndrome in which the body becomes the object of constant cognitive surveillance, every sensation is interpreted as evidence of serious illness, and the reassurance the patient seeks from doctors, internet sources, and family produces only temporary relief before the cycle restarts.

The brochure is for the reader who has noticed the pattern in their own life. The headache that becomes a brain tumour the article described. The mole that becomes a melanoma. The chest tightness that becomes a heart attack. The lymph node that becomes a lymphoma. The medical visit that reassures briefly. The new symptom the next week. The reader has noticed that the doctor's reassurance does not stay; that the medical-article reading worsens rather than relieves; that each test result produces a few hours or days of relief before something else surfaces. The pattern is what the brochure addresses.

A note before we go further. Health anxiety is sometimes confused with appropriate medical vigilance. Appropriate medical vigilance attends to symptoms that warrant evaluation, seeks evaluation when warranted, accepts the evaluation's conclusion, and returns to ordinary life. Health anxiety attends to all sensations as symptoms, seeks evaluation repeatedly, does not accept the evaluation's conclusion as durable, and does not return to ordinary life because the next symptom is always emerging. The distinction matters because the response is different. This brochure addresses health anxiety, not appropriate medical vigilance.

Three frames carry the health anxiety question.

The first frame is the structure of health anxiety. The clinical territory the brochure addresses.

The structure has several recurring features. The first is the constant scanning. «Psychosomatics Without Esoterica» names the pattern in its chapter on anxiety for health: the person constantly checks the body, looks for symptoms of illness, reads medical articles, finds new and new diagnoses. The cognitive baseline is one of monitoring rather than living. The body has become an object of surveillance rather than the medium of lived experience.

The second is the conflation of sensation with disease. The body produces sensations constantly — peristaltic activity, vascular pulsation, muscle tension, skin changes, temperature variation. Most of these are unremarkable. The mind that catastrophizes about health treats each as evidence of disease — the peristaltic activity becomes bowel cancer, the vascular pulsation becomes vascular disease, the muscle tension becomes a neurological condition. The baseline of bodily noise gets read as signal.
