For those who: have noticed that their anxiety produces bodily symptoms — palpitations, tremor, breath shortness, sweating, dizziness — and that the symptoms then become evidence the anxiety reads as confirming the threat. The reader has experienced the loop running on itself once started.

Not for those who: are experiencing cardiac, respiratory, or neurological symptoms whose substrate has not yet been examined by a clinician. Get the clinical evaluation first; the brochure addresses the loop pattern that persists after the substrate has been examined and ruled out.

What this is — the clinical reality

This is the ninth brochure of Bundle 8. The first eight addressed anxiety as a state, fear vs worry, the three-pencils method on anxious worry, rumination, catastrophizing, health anxiety, social anxiety, and anticipatory anxiety. This brochure addresses the anxiety-body loop — the bidirectional reinforcement between anxious cognition and bodily sensation. The mechanism by which the autonomic response to anxiety produces sensations that the cognitive system then reads as confirming the threat — and amplifies the response further.

The brochure is for the reader who has noticed the pattern in their own body. The first awareness of anxiety produces a heart-rate increase. The increase is noticed. The noticing produces the thought 'something is wrong with my heart' or 'I am going to panic'. The thought produces further autonomic activation. The activation produces more pronounced palpitations, possibly shortness of breath, possibly tremor or sweating. The expanded sensations confirm the original alarm. The loop runs on itself, without external trigger, until physiological exhaustion or cognitive recognition interrupts it. The reader has lived this pattern enough times to recognise it but has not had a name for it.

A note before we go further. The anxiety-body loop is not the same as the health anxiety pattern of Brochure 6, though the two often coexist. Health anxiety is the long-running cognitive surveillance of the body for evidence of disease. The anxiety-body loop is the short-running physiological feedback loop in which the body's response to anxiety becomes evidence the anxiety reads as confirming itself. The two have different timescales — the loop runs in minutes; the surveillance runs across years. The two interact, but the methods address different mechanisms. This brochure addresses the loop.

Three frames carry the anxiety-body loop question.

The first frame is the structure of the loop. The clinical territory the brochure addresses.

The structure has several recurring features. The first is that autonomic activation produces sensation. The anxious state, once triggered, activates the sympathetic nervous system — heart rate increases, breathing quickens, peripheral blood flow shifts, muscle tone rises, digestive activity reduces. Each of these produces sensation the conscious mind can detect: palpitations, breath shortness, cold hands, tremor, nausea. «Psychosomatics Without Esoterica» frames the underlying cascade: the amygdala detects threat — real or imagined — and acts fast, rapidly sending the signal to the hypothalamus, the dispatcher of the neuroendocrine system; the cost the body pays is the same whether the threat was actual or supposed.
