Advice on how to "fix" a narcissist or "build a healthy relationship" with one often leads nowhere. Dr. Saulitis makes a key point: before turning to any specialist, it is essential to understand what is actually happening — and with whom.

When "just talking it through" is not enough

Living with a narcissist frequently leaves a person in a state of chronic stress: thoughts spiral, anxiety mounts, self-blame grows. At this stage, a session with a psychologist or coach may not be sufficient. The doctor is direct about this: the first priority is restoring basic homeostasis — otherwise any work on "self-improvement" will be wasted effort.

"If the leg is broken, you need a cast first — you have to treat it first."

Why a psychiatrist specifically

A psychologist works with behaviour and thinking. But when the brain is running in overload mode — rumination, insomnia, anxiety, apathy — the root cause lies below the behavioural level. Dr. Saulitis insists: without understanding the actual state of the nervous system, any help risks becoming mere intellectualisation. A diagnosis is made by a medical specialist, and that is the starting point.

"To understand anything and move forward, you first need a diagnosis. A diagnosis is always made by a qualified physician."

Signs that a visit to a psychiatrist should not be delayed

  • Thoughts loop endlessly and cannot be stopped without great effort
  • Usual ways of calming down have stopped working
  • Sleep, appetite, or basic concentration are disrupted
  • Everything has been "explained to you," yet nothing improves

This is not weakness, nor is it an extreme case. It is a signal: the system needs a medical assessment, not only conversation.

What does not require a specialist

Dr. Saulitis draws a clear line: if a person is mentally well — with adequate inner resources — they can see the situation clearly and act on their own. A specialist is needed when that resource is gone: when fatigue, anxiety, or low mood prevent clear thinking and action.

Educational material. Not a diagnosis or a substitute for an in-person consultation; in an acute state, seek a doctor (emergency — 112).
